Do you have a medical history of type 2 diabetes mellitus and are you currently taking medications for treatment? *

Have you ever been diagnosed with Heart Failure, been hospitalized with heart failure, or currently receiving treatment for Heart Failure? *

1. Prior heart attack, Cardiac Bypass surgery, or Stent placement
2. History of stroke not caused by bleeding in the brain
3. History of carotid stenting or revascularization
4. History of peripheral artery disease as defined by:
     (i) Previous limb angioplasty, stenting, or bypass surgery
     (ii) Previous limb or foot amputation due to poor circulation
